Your Complimentary Consultation

We offer a complimentary one hour consultation with Dr. Zukowski. Usually these free plastic surgery consultations take place on Wednesdays from 6:00 AM to 2:00 PM (CST). We can accommodate you outside of this schedule if you have a work or school conflict. For out-of-town patients, the consultation is done on the phone.

How to Sign up for Your Free Consultation

  1. Submit the consultation form below
  2. Send photos of the area(s) you want to address. The views needed are outlined in the photo submission guidelines.
  3. We will reach out to you to request additional information and make an appointment for your free phone consultation with Dr. Zukowski.

Generally, if you respond to our requests for information in a timely manner, we are able to get you in to talk to Dr. Zukowski in as little as one week.

    I authorize all communication via e-mail, text, telephone and voice mail and I understand that all attempts will be made to ensure confidentiality of data between both parties (The Zukowski Center and myself). I also understand that all means of these communications can be potentially compromised with today’s technologies.

    Photo Submission Guidelines

    Try to find a neutral background in good light and send representative pictures in both your male and female personas (if this is for feminization purposes and you are not full time). If full time, send pictures of how you routinely dress on a day-to-day basis.

    Please read the following information and send an email containing photos to: contactus@mlzukowskiffs.com.

    1. Photos for facial procedures: We need facial photos in five views: Front, Right and Left profiles, and Right and Left at a 45-degree angle. We also need a close up forehead picture with the chin tilted downward. Finally, we need to see the base of your nose, looking into the nostrils with your chin tilted upward. Facial photos should be taken WITHOUT make-up and with hair TIGHTLY held back so we can clearly see the FRONT HAIRLINE.
    2. Facial and body photos must reflect your muscles in their relaxed state.
    3. Extra facial photos: If you are a full-time female, photos with and without make-up are helpful. If you are gender variant, photos in male and female personas are helpful.
    4. Upper body or breast photos should show the area from neck to waist. For breast augmentation we need two photos: (1) arms at the side in an “at attention” position, and (2) arms out at 90 degrees with the palms showing forward and the fingers together.
    5. Abdominal photos should be relaxed, not “sucked in” and show the area from the breast to the pubic bone in both a Front view and Right and Left side views.
    6. Back photos should show the area from the underarm to the tailbone with arms out at 45 degrees.
    7. Hip and buttock photos should show the area from waist to mid-thigh including the buttocks. UNDERWEAR CANNOT COVER THE BUTTOCKS. We need two 90 degree side view photos, and one photo from straight behind.
    8. Upper thigh photos should show the area from mid-hip to just below the knees. We need two 90 degree side view photos, one front-view photo, and one photo from straight behind.

    Note: Our email account is secure and HIPAA compliant. Your photos will never be shared with anyone outside of Dr. Zukowski and his staff in the course of assessing and treating you.

    After the Consultation

    Dr. Zukowski’s Executive Correspondence Secretary will then generate a letter to you within 7 to 10 days of your consultation. This letter outlines his recommendations and states the exact costs that were discussed with you during your consultation. We can expedite the letter if you are looking to schedule surgery sooner.
